Audiovisual Requests
Request catalog records for non-print materials that you have received as gifts or ordered direct to your library. It is still recommended that you send in music (photocopies okay)

Ordering Form
Request catalog records for print and non-print materials that you want to purchase. If you do not have an ISBN, please enter NO ISBN in the required field. This is not limited to Polaris Acquisition's users.

Corrections Form
Request updates to existing catalog records. This includes adding ISBNs, correcting typos, etc. Please be specific when requesting an update.

Magazine Form
Request serial records for magazines (print or non-print)
